You may either submit your Test Registration online (using the form below), or you may download and email the Test Registration form as a Word document.  Regardless of which method you choose, please be sure to answer all questions fully and clearly.

If you choose to use this web form, we strongly recommend that you review it thoroughly before beginning so that you know what information is necessary.  Once you begin, you cannot save your answers and return to finish another time.
